

A community health needs assessment gives organizations comprehensive information about the community’s current health status, needs, and issues. This systematic process involves the community to identify and analyze community health needs. In turn, this information can help with developing a community health improvement plan by justifying how and where resources should be allocated to best meet community needs.
A community health improvement plan is a long-term, effort to address public health problems based on the results of the community health assessment. The plan sets forth goals, objectives and potential strategies for addressing the priorities identified.
The most recent Community Health Needs Assessment was complete in 2022 with the following prioritiy areas to be address in the Community Health Improvement Plan.
The three community health priorities:
- Mental Well-Being
- Alcohol and other Drug Use
- Healthy Living & Active Lifestyles
