LONG TERM SUPPORT
Long Term Support services are provided to aging and disabled
residents who have the expectation of needing personal assistance on a
more-or-less permanent basis. The need for LTS services is generally the result
of a disability or the normal aging process. The goal of LTS services is to
allow the customer to remain in his or her home for as long as safely possible.
When it is impossible for a person to remain at home, services are provided in
the lease restrictive manner possible, such as the home-like setting of
relative, adult family home, or community based residential facility. If
someone is found to be both financially and medically eligible, the services
they need may be funded through the Long Term Support Programs.
What Can the Programs Provide?
Everyone's needs are different. The programs may fund
assistance with housekeeping, meal preparation, transportation, lawn care and
snow removal, modifications to a person's home to make it handicapped
accessible, Lifeline, ramps, and respite care for family caregivers. These
programs are intended to supplement, not replace services already being provided
by family members and other agencies.
There is no cost for an assessment of your needs or for the
development of a plan to meet your needs. Cost for services is based upon the
individual's ability to pay. Frequently, if the individual is eligible for
services, there is no cost to them. For more information on the Community
Options Programs:
